1 Applicant must be us citizen 16 years or older

Physical passport valid for 6 months or longer ,make sure you have signed your passport.

2 Never been issued and US passport



3 Applicant must be under 16 years old when most recent passport was issued

4 Applicant has not had an US passport issued for more than 15 years

5 Proof of citizenship

Birth Certificate or Certified Copy (long form),previous US Passport or US Naturalization Certificate.

6 Proof of identification

Valid Government issued ID or Driver’s License or Military ID issued at least 6 months ago.

7 Official passport photos

2 recent,identical color passport photos (no glasses may be worn).

8 Proof of travel

Flight Itinerary with departure within 2 weeks from US.
